  • Care Co-ordinator

    Sheffield

    The Care Coordinator’s primary responsibility is to administer care management and coordination activities for the clients of Intercare. This will include developing and monitoring care management and care coordination processes, efficiently and effectively.

    The main responsibilities of this role are:

    • To schedule the delivery of quality care to clients who receive support from our home Care Workers in their own home
    • To support the Care Worker team in delivering a high quality service.
    • To be aware of and comply with all Intercare policies and procedures in undertaking the role.
    • To ensure that all IT systems and processes in place and up to date
  • Care Assistant – North Sheffield

    North Sheffield

    As a Care Assistant you will work directly with our service users, providing care to a diverse range of people to their pre-agreed care plans. You will work on a rota basis, visiting several service users on each shift, so it is preferable that you have access to your own transport and hold a full UK driving license. You do not need to have previous experience to do well in this role but a caring manner and commitment to helping are very important to us. We provide a first-class induction and training programme, as well as an extensive supported induction package.
